Please include dates of birth and names for up to 4 children in the personalization box when ordering. For additional children please contact us. (There is an additional $2 for every additional date/name combination added.)

A mockup will be sent to you via Etsy for you to verify dates and spellings. You MUST approve the mockup before we will begin processing your oder. If you do not approve the mockup within 5 business days, your order will be cancelled and you will receive a full refund.
